Patellar instability
Patellar instability assessment centres on patellar height, measured as a ratio on a lateral radiograph, with patella alta above roughly 1.2 a recognised and correctable risk factor.
What you are actually deciding
Choose the index by what surgery the patient has had. Insall-Salvati references the tibial tubercle and is invalid after tubercle osteotomy; Caton-Deschamps references the tibial plateau and remains valid. Patellar height is one factor among several — trochlear dysplasia and TT-TG distance matter too.

- Caton-Deschamps Calculator2 itemsCaton-Deschamps IndexThe Caton-Deschamps index is the ratio of the distance from the inferior pole of the patellar articular surface to the anterosuperior corner of the tibial plateau, divided by the length of the patellar articular surface, where 0.6 to 1.2 is normal, above 1.2 is patella alta and below 0.6 is patella baja.
- Insall-Salvati Calculator2 itemsInsall-Salvati IndexThe Insall-Salvati index is the ratio of patellar tendon length to patellar length measured on a lateral knee radiograph; a ratio between 0.8 and 1.2 is normal, above 1.2 indicates patella alta, and below 0.8 indicates patella baja.
